The unlikely origin

II studied Industrial Engineering at The University of Pretoria (TUKS). Then I spent years as a software developer — building systems, solving problems, thinking in logic and loops. At some point I got bored of building things that didn't talk to markets. I wanted to understand what made people choose one thing over another. So I taught myself advertising. And then something clicked that most marketers never find: the principles I'd used to optimise factory floors and software systems worked almost identically on Google Ads campaigns.

Constraints. Throughput. Waste elimination. Build-measure-learn. The Theory of Constraints applied to a conversion funnel the same way it applied to a production line. I wasn't guessing at advertising — I was engineering it.That's how LeanAds started. E-commerce was the main focus. Shopify brands, product catalogues, ROAS targets. That's where the big contracts were. That was the plan.

Then my phone started ringing from a different direction. A family member — a doctor — mentioned casually over dinner that their practice was quiet. "People just don't know I exist," she said. It wasn't a pitch. It wasn't a brief.

It was just a conversation. I said I'd take a look. No expectations. Just curiosity.

What I found surprised me. The practice was exceptional — skilled, caring, exactly what patients needed. But online? Invisible. No Google presence. No ads. Nothing. Meanwhile, a competing practice two streets away — frankly not in the same league clinically — was appearing at the top of Google for every relevant search in the area.

I built a simple campaign. Search ads. Google Maps. A contact form. Nothing complicated. Within six weeks, her phone hadn't stopped ringing.

She told me — and I remember this exactly — "I've had to start a waiting list. I've never had a waiting list." I didn't think much of it at first. One good result. Maybe it was the market. Maybe it was timing.

Then a friend. Another doctor. Same story — brilliant clinician, empty slots, invisible online. Same approach. Same result.

Then another. And another.

What started as favours for people I cared about started becoming the most consistent result set I'd ever seen across any vertical. The e-commerce campaigns I was running — with ten times the budget, ten times the complexity — weren't producing results this clean. These practices were simple to optimise. High intent searches. Low competition. A patient with a specific problem, looking for a specific solution, and a good doctor ready to help them. I started paying attention.

The pattern across every practice was the same: the quality of care was never the problem. The discoverability was. These doctors had spent a decade becoming exceptional at what they did — and then trusted that reputation alone would fill their rooms. For a while, it did. But the world had moved. Patients weren't asking their GP for a referral and waiting. They were Googling at 11pm and booking the first credible result they found.

The practices winning weren't always the best ones. They were the ones who showed up.
That realisation changed the direction of LeanAds.

E-commerce remains part of what we do. But private practices are now a core focus — because nowhere else have I seen the gap between quality and visibility this wide, or the return on closing that gap this fast.

The underdog vertical turned out to have the most to gain. And the doctors I've worked with — some of whom now run waiting lists for the first time in their careers — have become the most satisfying client work I've ever done.
